Tax Forms and Tax Help

This guide will help you get the state and federal tax forms and information you need to file your taxes. It also points you to sources of free tax help.

Where to Get Federal Tax Forms

The Boston Public Library does have tax forms.

Tax forms are available at the Central Library and most branches. If you are hoping to get them from a branch, call ahead to make sure they are in stock.

Forms available are the IRS 1040 booklet and the instruction booklet for those forms along with the Massachusetts Form 1 and Form 1 NR/PY (for nonresident and part year resident filers.)

Federal Forms (IRS)

Online – Federal forms and instructions can be downloaded at www.irs.gov/forms. There is also the option of electronically filing a return, either through free software (income requirements) or fillable online forms (no income requirements). Another way to find online tax forms quickly is to type "form [number]" into your browser. When you see your search results be careful to download the form for the 2019 tax year.
By Phone – Call the IRS at 1-800-829-3676
By Mail – IRS forms can also be mailed to you. Order them here. Allow 7-10 business days to receive your forms.

State Forms

By Phone – Call the Massachusetts Department of Revenue at 1-800-392-6089
